Dr Ion Bogdan Codorean announces the publication of his latest scientific paper in the prestigious journal Arthroscopy Techniques, one of the most important international publications dedicated to arthroscopic surgery and sports medicine. The paper presents an innovative surgical technique for anterior cruciate ligament (ACL) reconstruction, developed to provide superior knee stability and faster recovery, particularly in competitive athletes and patients with high functional demands.
A modern approach to anterior cruciate ligament reconstruction
An anterior cruciate ligament tear is one of the most common injuries encountered in orthopaedics and sports traumatology. Although ACL reconstruction is a standardised procedure, the choice of graft and the reconstruction technique significantly influence knee stability, the patient's recovery and the possibility of returning to sport.
In this paper the authors describe a modified all-inside reconstruction technique using a 4×2 hybrid graft, formed by combining a tubular synthetic device (JewelACL) with the patient's own gracilis tendon. Compared with classic techniques, this configuration allows a thicker and biomechanically stronger graft to be obtained, while preserving as much as possible of the patient's own anatomical structures.
The main advantages of the technique
The proposed technique aims to combine the benefits of a biological graft with those of a modern synthetic implant. The advantages highlighted by the authors include:
- increased biomechanical strength immediately after the operation;
- preservation of the semitendinosus tendon, using only the gracilis tendon;
- reduced surgical trauma through the all-inside technique, which preserves a greater quantity of bone;
- the possibility of faster functional recovery and an earlier return to sport;
- reduced operating time and less morbidity associated with harvesting several tendons.
Promising results
The authors present the experience accumulated in using this technique in more than 80 patients since 2023. Assessments carried out after surgery showed very good integration of the hybrid graft. At 12 months, second-look arthroscopy demonstrated the formation of ligament-like tissue around the synthetic implant, and MRI scans at 8 months confirmed correct positioning, continuity and appropriate maturation of the new ligament, with no signs of synovitis or widening of the bone tunnels.
The impact on sports surgery
The new technique is intended particularly for competitive athletes and active patients, for whom immediate knee stability and a reduced recovery time are essential objectives. By combining the mechanical support of the synthetic implant with the biological potential of the autologous tendon, the authors propose a modern solution that can help optimise the results of anterior cruciate ligament reconstruction.
